(A) Any person who witnesses or has personal knowledge that an act or acts made unlawful by this subchapter have been committed may sign a complaint against the alleged violator.
(B) Any police officer, animal welfare officer or code enforcement officer who is employed by the city is authorized to issue a summons and complaint when the officer personally observes a violation of this subchapter.
(C) The complainant must provide a sworn complaint to the officer receiving the complaint containing the following information:
(1) Name, address and telephone number of the complainant and other witnesses to the incident;
(2) Date, time and location of the incident;
(3) Description of the animal;
(4) Name, address and telephone number, if known, of the animal owner;
(5) A statement that the animal attacked the complainant or some other person or animal as witnessed by the complainant or the facts as warrant a finding that the animal is vicious, dangerous or diseased; and
(6) Other facts and circumstances of the incident.
