§ 151.02.04 RENEWAL OF PERMIT; FEE.
   (A)   On or before the anniversary date of the issuance of the permit each year, permittee shall file an application for renewal of the base permit previously granted. Such application shall include the following:
      (1)   A renewal fee in an amount set forth in the Manual of Fees.
      (2)   A statement of amendments as to any factual issues which have been changed from the facts set forth in the initial application.
      (3)   As to wells which were drilled during the previous year, a statement as to whether the well has been converted to production, and if so, identifying the strata and depth of production.
      (4)   Evidence that all bonds and insurance remain in force or have been replaced with equivalent new bonds and insurance.
      (5)   A statement of the safety history of the well during the past year, which must include a summary of each type of claim submitted, name of the claimant(s), the amount of the claim(s), and the amount(s) of settlement (if any). Provided that claims, as used in this section, shall not include claims involving worker's compensation, but shall include spills on the land, contamination of subsurface strata, spills of oil, gas, or deleterious substances which occurred within the city limits in route to or from the well in question.
      (6)   The application for renewal may be submitted through the mail or in person. Failure to make the required application for renewal shall suspend all rights to drill, produce, inject or dispose, but shall not be construed to terminate the base permit until after the time required to make the well an abandoned well. Any operation conducted while the permit rights are suspended shall not prevent the well from becoming an abandoned well, all such operations are offenses and forbidden until the renewal is issued.
      (7)   The city shall notify permittee by letter within 30 days thereafter that the base permit has been extended, or that the base permit has not been extended, for a period of one year. This notice of extension shall be maintained by the permittee with the permit and used as a supplement thereto for all purposes. In the event the permit is not renewed and expired, the permittee shall immediately cease all drilling, production operations and diligently proceed to plug the well.
