EXHIBIT C   CITY OF BERWYN INTERESTED PARTIES REGISTRY REGISTRATION FORM FOR ORGANIZATIONS (NON-INDIVIDUALS)
Please read and review the TIF Interested Party Registry Rules. If you are eligible to register as an organization, complete the following application form. In addition, you must complete a one-page statement describing the organization’s current operations and activity within the municipality. Submit the completed form, the one-page statement, and a copy of your current driver’s license or state identification card (to confirm proof of address) to the City Clerk of the City of Berwyn, 6700 West 26th Street, Berwyn, IL 60402. After the City Clerk of the City of Berwyn receives this application, you will be contacted by mail within ten days after the receipt of this application.
