Prior to any employee or agent of a valet parking operator driving or parking any motor vehicle, the valet parking operator shall submit the name, address and driver’s license number, on a form to be approved by the Police Department, along with the written consent of the employee or agent permitting a check of his or her driving record to determine if his or her driver’s license is currently valid.
