(A) Application for a license, pursuant to this section, shall be made to the city on forms provided by the city. In addition to the license application provisions of Chapter 801, an application for the license required in § 829.03 hereof shall contain the following information:
(1) Name and address of the applicant;
(2) The specific location of the proposed automated rental machine; and
(3) The number and description of the automated rental machine intended to be kept for use at that location.
(B) The applicant must also provide, accompanying the application, proof of city electrical permit, a certificate of insurance and a statement of permission to locate the automated rental machine signed by the owner of the property, business owner or representative thereof, where the automated rental machine will be located.
