In addition to the license application provisions of Chapter 801, an application for the license required in § 818.01 shall include the name, residence, citizenship and employment or business of the applicant during the previous five years; whether or not the applicant was ever convicted of a felony or a misdemeanor; whether or not the applicant owns real estate; and the value of the same. The application shall also contain three references who are residents in the city.
