§ 804.03 LICENSE APPLICATIONS.
   An application for the license required in § 804.02 shall be made to the Liquor Control Commissioner, in writing; signed by the applicant, if the applicant is an individual, or by a duly authorized agent thereof, if the applicant is a club or corporation; verified by oath or affidavit; and contain the following statements and information:
   (A)   The name, age and address of the applicant (in case of an individual) or the persons entitled to share in the profits thereof (in case of a copartnership) or (in case of a corporation) the objects for which the corporation was organized, the names and addresses of the officers and directors and, if a majority interest of the stock of the corporation is owned by one person or his or her nominee, the name and address of the person;
   (B)   The citizenship of the applicant and his or her place of birth and, if the applicant is a naturalized citizen, the time and place of his or her naturalization;
   (C)   The character of business of the applicant. If the applicant is a corporation, a copy of the articles of incorporation thereof must be attached;
   (D)   The length of time the applicant has been in a business of that character or, in the case of a corporation, the date when its charter was issued;
   (E)   The amount of goods, wares and merchandise on hand at the time the application is made;
   (F)   The location and description of the premises or place of business which is to be operated under the license;
   (G)   A statement as to whether or not the applicant has made an application for a similar or other license on a premises other than the one described in this application and the disposition of the other application;
   (H)   A statement that the applicant has never been convicted of a felony and is not disqualified to receive a license by reason of any matter or thing contained in this chapter, the laws of the state or the ordinances of the city;
   (I)   Whether or not a previous license by any state or subdivision thereof, or by the federal government, has been revoked and the reasons therefor;
   (J)   A statement that the applicant will not violate any of the laws of the state, or the United States, or any ordinance of the city in the conduct of his or her place of business;
   (K)   The premises for which the license is sought are expressly zoned to allow for the sale of alcoholic liquor; and
   (L)   The names and addresses of the owners of record of the real estate housing the licensed premises, including the names of the true beneficiaries of any land trust holding legal title to the real estate.
