4-1-4: APPLICATION FOR LICENSE:
Applications for all licenses required by this chapter shall be made to the village clerk or designees in writing on a form prepared by the village. If the applicant is an individual (sole proprietorship), the application shall contain his name, date of birth, residential address, and residential telephone number. If the applicant is a partnership or other noncorporate business entity, the application shall contain the name, date of birth, and residential address and residential telephone number of each partner, principal, or member. If the applicant is a corporation, the application shall contain the name, date of birth, and residential address of each principal officer and the registered agent thereof. Each application also shall contain:
   A.   A designation of the type or kind of license desired;
   B.   The location or proposed location of the commercial establishment or activity;
   C.   The applicable fee to be paid including any village costs for outside consultant review. All fees shall be paid prior to the issuance of the license;
   D.   The number of the certificate of registration required under the retailers' occupation tax act 1 , service occupation tax act 2 , and/or use tax act 3 , if applicable; and
   E.   Such additional information as may be needed for the proper guidance of village officials in the evaluation of such application including, but not limited to:
      1.   Emergency contact person and/or key holder for the building.
      2.   Proof of any insurance policy or bond if required.
      3.   Identity of management employees and their position with the applicant.
      4.   Applicant's businesses in other communities.
      5.   Whether the applicant has ever had a previous license denied, revoked, or suspended.
      6.   Whether the applicant has ever voluntarily withdrawn a license or application for a license during or pending an investigation for the suspension or revocation of that license.
      7.   Whether the applicant has ever been arrested or convicted of a misdemeanor or felony.
      8.   Proof of corporate good standing.
   F.   Any change to the information provided in the application shall be reported to the village clerk within fourteen (14) days of the change. (2016 Code)
