250.08   SICK LEAVE.
   (a)   Every full-time employee is entitled to receive a credit of 4.6 hours of sick leave, with pay, for each completed 80 hours of service, excluding overtime. An employee is eligible to use sick leave upon completion of 80 hours of service.
   (b)   An employee shall file a written request for sick leave on forms provided by the Village. Sick leave is to be approved by the Department Head and the Mayor. Subject to such approval, employees may use sick leave for absence due to:
      (1)   Illness of the employee;
      (2)   Injury of the employee;
      (3)   Exposure to contagious disease which could be communicated to other employees; or
      (4)   Illness in the employee's immediate family (parent, spouse, child).
   (c)   A certificate from a physician may be required by the Mayor, or Department Head for any absence extending beyond three days. The Mayor may require that the employee submit to an examination by a physician selected by the Mayor, at the Village's expense, for any absence.
   (d)   An employee retiring who meets the age and service requirements of the applicable pension system or an employee who dies during active service with the municipality and has been employed with the municipality for ten or more continuous years prior to retirement or death may receive his or her accumulated sick leave.
   (e)   For employees hired on or before May 1, 1991, sick leave can be accumulated up to a maximum amount of 3,000 hours for conversion upon retirement or death. There shall be no maximum accumulation for use of sick leave. Redemption of sick leave hours shall be at the rate of one hour of pay for every four hours of accumulated sick leave. Said pay shall be calculated by combining the employee's rate of pay at the time of hire with the rate of pay at time of retirement divided by two and multiplied by his or her accumulated sick leave hours. The maximum conversion is one-fourth of 3,000 hours or a maximum pay equivalent of 750 hours.
   (f)   For employees hired after May 1, 1991, sick leave can be accumulated up to a maximum of 960 hours for conversion upon retirement or death. There shall be no maximum accumulation for use of sick leave. Redemption of sick leave hours shall be at the rate of one hour of pay for every four hours of accumulated sick leave at the employee's last rate of regular pay prior to such retirement. The maximum conversion is one-fourth of 960 hours or a maximum pay equivalent of 240 hours.
