§ 150.19 PERMIT VALUATION AND FEES.
   (A)   Fees shall be as provided by duly adopted schedules. Fees will be set based upon the dollar value of the construction for which the permit is to be issued. The estimated cost of construction shall be determined by the Building Commissioner based on the following:
      (1)   Commissioner may accept an estimate furnished to him or her as provided by the applicant;
      (2)   Estimate of cost based upon published cost figures for like, projects in the immediate Chicago Metro area;
      (3)   Certificate of cost from a licensed architect, registered structural engineer or other recognized registered design professional.
   (B)   The Commissioner may require and accept a sworn affidavit from the owner or owner's agent. accompanied by a copy of the contract for the construction for which the permit is to be issued.
   (C)   Any combination of the above. If divisions (A)(1), (A)(2), or (A)(3) above are used the method that establishes the cost as the least expensive shall apply. Fees to be charged shall be in accordance with the attached schedule.
   (D)   A non-refundable $50 application and review fee shall be paid at the time of submittal of the application.
   (E)   Third party review fees accrued as part of a proposed project shall be paid on a monthly basis when the project approval is expected to last more than 30 days.
   (F)   Construction costs of $600 or less may be charged a permit fee of 10% of the cost of construction with a minimum permit fee of $25 at the Building Commissioner’s discretion.
   (G)   New construction.
      (1)   Accessory buildings for one-, two- and three-family dwellings.
         (a)   Basic fee for first 1,000 square feet: $150;
         (b)   Per each 100 square feet or fraction: additional $36;
         (c)   Slabs, if poured separate: $50;
         (d)   Plan review, bonds, licenses as applicable.
      (2)   One-, two- and three- family dwellings.
         (a)   Basic fee for first 1,000 square feet: $312;
         (b)   Per each 100 square feet or fraction: additional $56;
         (c)   Plan review, bonds, licenses as applicable.
      (3)   Four-family and up, non-residential buildings and associated buildings.
         (a)   Basic fee for first 1,000 square feet: $380;
         (b)   Per each 100 square feet or fraction: additional $75.
      (4)   For buildings with fire detection and/or suppression systems.
         (a)   Per 100 square feet or fraction thereof: plus $16;
         (b)   Plan review, bonds, licenses as applicable.
   (H)   Alterations and additions.
      (1)   Accessory buildings.
         (a)   Basic fee for first 500 square feet: $75;
         (b)   Per each 100 square feet or fraction: additional $18;
         (c)   Slabs, if poured separate: $50;
         (d)   Plan review, bonds. licenses as applicable.
      (2)   One-, two-and three-family dwellings.
         (a)   Basic fee for first 500 square feet: $165;

      (3)   Four-family and up, non-residential buildings.

      (1)   Construction or demolition water shall be paid as per the established village water rate.
      (2)   Meters may be purchased from the village or obtained from the village in return for a cash deposit.
      (3)   Deposit fee to be determined by the Superintendent of Public Works.
   (L)   Roofing.
      (1)   Residential: $75; accessory building (garage): $40;
      (2)   Commercial: $100 for the first $1,000 of construction plus $25 for each $1,000 or a portion of $1,000 above the initial $1,000.
      (3)   Plan review, bonds, licenses as applicable.
   (M)   Sheet metal work.
      (1)   $75; plus
      (2)   Plan review, bonds, licenses as applicable.
   (N)   Miscellaneous construction - shoring, raising, moving of any building.
      (1)   Basic fee for first 2,000 square feet: $126; plus
      (2)   Per 100 square feet or fraction. $25;
      (3)   Bonds, licenses as applicable.
   (O)   Canopy, fixed awning or alterations.
      (1)   Basic fee: $75; plus
      (2)   Annual use fee: $45;
      (3)   Bonds, licenses as applicable.
   (P)   Tanks, raised or underground.
      (1)   Basic fee for first 2,000 gallon: $125;
      (2)   Per additional 1,000 gallon capacity or fraction thereof, for underground tanks: $30;
      (3)   Structural supports for raised tanks over 400 gallon capacity: $75;
      (4)   Bonds, licenses as applicable.
   (Q)   Elevators, escalators, lifts, hoists, dumbwaiters, conveyors.
      (1)   Basic fee for first five floors: $175;

      (7)   Inspection of one- to three-family dwelling prior to sale:
         (a)   Single-family: $175.

         (d)   Sales conducted without a presale inspection will be subject to a 100% increase in fees and a citation for each unit in the building.
      (8)   Inspection of food establishments upon sale, exchange of ownership:
         (a)   Food service establishments: $275.
         (b)   Retail food establishments: $275.
         (c)   Sales conducted without a presale inspection will be subject to a 100% increase in fees and a citation for each unit.
      (9)   Inspection of commercial property prior to sale:
         (a)   Under 5,000 square feet: $205.
         (b)   5,001 square feet to 9,999 square feet: $275.
         (c)   10,000 square feet and larger: $350.
      (10)   Annual fire inspection fees:
         (a)   Required annual fire inspections first inspection: no charge.
         (b)   First re-inspection to verify compliance of identified violations during annual fire inspection, or due to missed appointment for annual inspection: no charge.
         (c)   Second re-inspection to verify compliance of identified violations during annual fire inspection, first re-inspection, or due to missed appointment(s) for annual inspection or re-inspections: no charge.
         (d)   Third re-inspection and all re-inspections following: $50 per event.
   (HH)   Zoning filing fees. Per schedule maintained by Village Clerk.
   (II)   Plan review fees. Plan review fee for the examination of plans will be as follows:
      (1)   Basic fee alterations and repairs (minor); accessory buildings and all other work not addressed below as having a review fee: $50. This is included in the application and review fee paid at time of application. All other review fees shall apply as an additional fee to receive the permit.
      (2)   Basic review fees for new construction, additions, multi-family, commercial and industrial roofs, sprinkler and alarms and similar work listed below: $165 or 10% of permit fee whichever is higher. Or the fee charged by a third party.
      (3)   Review of revised plans (per page): $50.
      (4)   Fire suppression and/or detection systems and all commercial, industrial and multi-family structures three units and above shall be reviewed by the Village of Bellwood Fire Department and the review fees charged as shown above.
      (5)   Public health review of food handling systems and establishments shall be reviewed by the Cook County Health Department and the review fees charged as per the contract.
      (6)   Plan review fees are non-refundable.
      (7)   Plans sent to outside agencies: Plan review fees paid by the village shall be passed on to the applicant with a $225 add on or 10% above the cost to the village whichever is greater.
      (8)   Refunds:
         (a)   No fees will be refunded when a permit has lapsed after the work has started.
         (b)   When a permit is revoked at the request of the applicant prior to lapsing due to time limits, and no work has been done,. fees may be refunded minus a 10% fee of the total permit fees not including the plan review fee and plan review fees.
