§ 122.30 ADDITIONAL SUBMITTAL REQUIREMENTS.
   In addition to the information required elsewhere in the zoning ordinance, development applications for a personal wireless telecommunications facility, antenna support structure or tower shall include the following supplemental information:
   (A)   The name, address and telephone number of the owner and lessee of the parcel of land upon which a personal wireless telecommunications facility, tower or antenna support structure will be located. If the applicant is not the owner of the parcel of land, the written consent of the owner shall be provided with the application;
   (B)   The legal description and address of the parcel of land upon which the personal wireless telecommunications facility, tower or antenna will be situated;
   (C)   The names, addresses and telephone numbers of all property owners of property upon which other personal wireless telecommunications facilities, towers or antenna support structures are located within a 1/2 mile radius of the proposed new personal wireless telecommunications facility site, including property owned by a local governmental entity;
   (D)   A statement of the applicant’s purpose and need;
   (E)   Existing adjacent facilities, including existing and estimated capacities;
   (F)   A site plan, including tower and equipment elevations indicating how visual impacts will be minimized;
   (G)   Demonstration from a qualified and licensed professional engineer that alternative locations are unavailable or impractical and that the equipment cannot be mounted on an existing tower;
   (H)   A report from a qualified and licensed professional engineer which:
      (1)   Describes the tower height and design, including a cross section and elevation;
      (2)   Documents the height above grade for all potential mounting positions for collocated antennas and the minimum separation distances between antennas;
      (3)   Describes the tower’s capacity, including the number and type of antennas that it can accommodate;
      (4)   Documents what steps the applicant will take to avoid interference with established public safety telecommunications;
      (5)   Includes an engineer’s stamp and registration number; and
      (6)   Includes any other information necessary to evaluate the request;
   (I)   For all personal wireless telecommunications facilities and towers, a letter of intent committing the owner and his or her successors to allow the shared use of the tower if an additional user agrees in writing to meet reasonable terms and conditions for shared use;
   (J)   In order to assist in evaluating visual impact, the applicant shall submit color photo simulations showing the proposed site with a photo-realistic representation of the proposed tower or antenna as it would appear viewed from the closest adjacent property and roadways;
   (K)   An affidavit attesting to the fact that the applicant made diligent but unsuccessful efforts to install or collocate the application's personal wireless telecommunications facility on towers or antenna support structures located within a 1/2 mile radius of the proposed facility site;
   (L)   Written, technical evidence from an engineer that the proposed structure meets the structural requirements of the zoning ordinance, including all requirements of this chapter;
   (M)   Written, technical evidence from a qualified engineer acceptable to the Fire Marshal and the Building Official that the proposed site of the tower or personal wireless telecommunications facility does not pose a risk of explosion, fire or other danger to life or property due to its proximity to volatile, flammable, explosive or hazardous materials such as LP gas, propane, gasoline, natural gas or corrosive or other dangerous chemicals;
   (N)   A written statement from an engineer that the construction and placement of the personal wireless telecommunications facility will not interfere with public safety communications and the usual and customary transmission or reception of radio, television or other communications services enjoyed by adjacent residential and nonresidential properties.
