Every person desiring a permit to drive a taxicab shall make application in writing therefor to the Village Comptroller. Such application shall set forth the following:
(A) The name, including the middle initial, of the applicant;
(B) Applicant's date of birth;
(C) Applicant's address of his or her residence;
(D) Applicant's state license number;
(E) Applicant's employer;
(F) An affidavit from the applicant swearing or attesting to the following:
(1) Applicant is 21 years of age or older.
(2) Applicant has more than one year of driving experience.
(3) Applicant possesses a valid license for the purpose of transporting persons for compensation in a motor vehicle issued by the Illinois Secretary of State.
(G) A current photograph, two inches by two inches, clearly depicting the frontal face view of the applicant;
(H) Applicant's fingerprints;
(I) A list of all convictions against the applicant under the Illinois Vehicle Code;
(J) A list of all criminal convictions against the applicant.
