All applications for a license under this chapter shall be made in writing, on forms provided for that purpose, to the Commissioner, signed by the applicant. Every application must be submitted and filed with the Commissioner at least 15 days prior to the issuance of any license, and shall contain the following information and statements:
(A) The name, age, Social Security number, driver's license number, state of birth and current residence address of each individual required to sign the application;
(B) In the case of a business or not-for-profit corporation, the objects for which it was formed, the date and place of incorporation, and the names, ages, Social Security numbers, driver's license numbers, dates of birth, and current residence addresses of every officer, director, shareholder holding 5% or more of stock, or member;
(C) The citizenship of the applicant, his or her place of birth and if a naturalized citizen, the time and place of his or her naturalization;
(D) The character of the business of the applicant;
(E) The length of time that the applicant has been in business of that character;
(F) The location and description of the premises or place of business which is to be operated under such license, and a statement that the applicant owns such premises or has a lease thereon for the full term for which the license is requested;
(G) The class of license applied for by the applicant;
(H) The nature of any entertainment proposed to be offered on the premises to be licensed;
(I) A statement as to whether food is to be sold on the premises to be licensed;
(J) The length of time that the applicant has resided within the village; or in the case of a general or limited partnership or corporation, the length of time that each partner has resided within the village; or in the case of a business or not-for-profit, the length of time that a proposed resident manager has lived in the village;
(K) A list of all governmental entities to which applicant has submitted an application for a liquor license, the dates on which such applications were submitted, the disposition of such applications, the amount of and reason for any fine imposed upon applicant under any other liquor license held and that dates, reason and length of any suspension or revocation of any other liquor license held by the applicant;
(L) A list of all convictions of the applicant for all offenses, other than a violation of any traffic law of this or any other state, except that applicant shall also disclose any traffic conviction involving in any manner drugs or alcoholic liquor and indicating the nature of the offense, date of conviction and disposition;
(M) A statement that the applicant will testify under oath and answer all competent, relevant and material questions propounded at any hearing conducted by the Commissioner, or his or her designated representative, either before or after the issuance of a license to him and that his failure to so testify shall be sufficient reason to refuse to issue or renew any license or to suspend or revoke any license that has been issued;
(N) A statement that the applicant will not violate any of the laws of the state or the United States, or any ordinance of the village, in the conduct of the applicant's business;
(O) Response to all questions asked of applicant on any forms provided to the applicant by the village;
(P) A copy of the applicable partnership agreement, articles of organization, or articles of incorporation;
(Q) For any leased premises, a copy of the lease and any assignment, sublease or extension;
(R) A completed criminal background and credit check authorization form;
(S) A copy of any assumed name certificates applicable to the applicant's business;
(T) In the case of a foreign corporation, a copy of its certificate of authority to operate in the state;
(U) A copy of the applicant's certificate of registration under the Retailers' Occupation Tax Act;
(V) A certificate of insurance documenting liquor liability insurance coverage for the licensed premises in an amount that is at least equal to the maximum liability amounts set forth in Article VI of the Illinois Liquor Control Act, ILCS Ch. 235, Act 5, § 6-12(a), as amended;
(W) The names of all individuals who are responsible for managing the establishment at which alcoholic liquor will be sold;
(X) Copies of any other documents requested by the Commissioner to report or clarify information contained in the application or any document submitted with the application.
