§ 110.01 APPLICATION; FILING AND CONTENT.
   (A)   Applications for all licenses and permits required by this code or ordinances of the village shall be made in writing to the Village Comptroller in the absence of provision to the contrary.
   (B)   Each application shall state:
      (1)   The name of the applicant;
      (2)   The permit or license desired;
      (3)   The location to be used, if any;
      (4)   The time covered;
      (5)   The fee to be paid; and
      (6)   Such additional information as may be needed for the proper guidance of the village officials in the issuing of the permit or license applied for.
   (C)   Each application shall also include an executed consent form giving the village access to sales tax information reported by the business to the State of Illinois for use in conducting any audit under the Locally Imposed and Administered Tax Rights and Responsibilities Ordinance, §§ 35.155 through 35.169.
