The owner or agent responsible for the property shall, on a form provided, request the presale inspection and provide, with the completed form, a copy of a current plat of survey of the property prepared by a licensed surveyor. Information to be provided shall include the actual name and address of the owner and agent of the property as well as other such information as required to determine specific responsibilities.
(A) Service companies. The application form shall include the following information as may apply to the specific property:
(1) Fire alarm - Contractor or testing company.
(2) Elevators - Service company or testing agency.
(B) Written consent to inspect. The owner or agent responsible shall indicate by signature that the village or its' contractors shall be permitted to enter all areas of the property or structure in order to complete the terms of the inspection.
(C) Test results:
(1) Fire alarm; or
(2) Emergency lighting system.
