(A) At least 21 days prior to a motor vehicle's state registration number being placed on an immobilization list, the Chief of Police, or his or her authorized officer, shall cause a notice of said designation to be delivered personally or mailed by certified mail and regular mail to the last registered owner of the vehicle, at the address recorded with the Illinois Secretary of State, or such other state's registry of motor vehicles.
(B) The notice shall state the name and address of the registered owner, the state registration number of the vehicle, the nature of the ordinances violated, and the number and issue dates of the outstanding citations.
(C) The notice shall also advise that the owner may request a hearing at the date, time, and place as specified in the notice, on the question of whether the motor vehicle shall be placed on the immobilization list by appearing in person and by submitting evidence to disprove that the vehicle is eligible for immobilization.
(D) A request for hearing shall be returned to the Chief of Police within 14 days of the date of the notice. The Chief of Police shall then set the matter for hearing at the date, time, and place as previously stated in the notice.
