The purpose of the Accident Review Board is to determine if a Bellwood employee and/or a motor vehicle, truck, mobile or portable equipment was involved in a preventable or non-preventable accident not at fault, partially at fault, or at fault. The Board shall review every accident as mentioned and shall submit a finding of preventability/nonpreventability and not at fault, partially at fault, or at fault on the appropriate form to the department head. The Board shall submit to the department head an annual report summarizing the year's accidents/incidents.
