§ 27.5-84 REVOCATION.
   (A)   Any permit issued under authority of this chapter may be revoked at any time whenever the holder of any such permit, knowingly or otherwise, violates the conditions of such permit or any provision of this chapter or any related laws.
   (B)   Examples of such violations include:
      (1)   Failure to report significant changes in operations, or stormwater constituents and character;
      (2)   Refusal of reasonable access to the user’s premises for the purpose of inspection or monitoring;
      (3)   Violation of conditions of the permit; or
      (4)   Falsification of information on the permit application or in subsequent reports.
