Any distributor desiring a permit required by the provisions of this article shall make application therefor to the city clerk, which application shall contain:
(A) Name and address of the applicant, age, date and place of birth.
(B) The location of his office or business.
(C) The number of machines or devices to be distributed within the limits of the City of Bellevue.
(D) A list indicating the type of machine, whether it is mechanical or electronic video amusement, tobacco vending, automatic musical, or merchandise vending, and the address of each machine, provided that if any machine is located in a building or facility with more than a single floor or place of business, the list shall specify where in such building or facility the machine is located. The list shall be regularly supplemented on a semiannual basis to reflect any additional machines, deletions or changes in the locations of any machine.
