(A) Written application for a license required by the provisions of this article shall be made to the authority, or its designee, and the applicant shall:
(1) State the name and address of the owner of the pot-bellied pig;
(2) State the color, age and sex of the pot-bellied pig;
(3) Submit documentation signed by a licensed veterinarian indicating that, upon reaching the age of four months, the pot-bellied pig has been neutered or spayed; and
(4) Provide such other information as may identify the pot-bellied pig.
(B) The applicant shall certify to the information contained in such application under penalty of law for the willful making of any untrue statement.
