(A) Before any such license shall be granted by the city, the applicant therefor shall file an application in the office of the city clerk.
(B) The following information shall be furnished by the applicant upon the form furnished by the city clerk:
(1) Description and location of the place where the activity is to take place;
(2) What provisions have been made for sanitary facilities, running water, power, and parking;
(3) The day or days upon which the event is to take place;
(4) The times during which it is to take place; and
(5) The name and address of the person(s) requesting the permit, and the corporate officers and their names, if the applicant is a corporation.
