There shall be a maximum fee not to exceed fifty-five cents ($.55) per access line per month for the first eighteen (18) months of installation; a maximum fee not to exceed three dollars and thirty cents ($3.30) per access line per month for the initial charge thereafter, with neither charge to exceed the total cost of twenty-five (25) access lines per month on each monthly telephone bill.
