Any SIU who violates the following conditions of this chapter, or applicable state and federal regulation, is subject to having his wastewater contribution permit revoked in accordance with the procedures of Section 925.32(a)(1):
(a) Failure of the SIU to factually report the wastewater constituents and characteristics of his discharge;
(b) Failure of the SIU to report significant changes in operations, or wastewater characteristics and constituents;
(c) Refusal of reasonable access to the SIUs premises for the purpose of inspection, sampling or monitoring;
(d) Violations of the conditions of the permit.
