925.37 INDUSTRIAL PRETREATMENT WASTEWATER CONTRIBUTION PERMITS.
   (a)    All significant industrial users (SIU) proposing to connect to or to contribute to the sewage system shall obtain a wastewater contribution permit before connecting to or contributing to the sewage system. All existing SIUs connected to or contributing to a sewage system shall obtain a wastewater contribution permit within 180 days after the effective date of this chapter.
   (b)    Users required to obtain a wastewater contribution permit shall complete and file with the City an application in the form prescribed by the City and accompanied with the processing fee of twenty-five dollars ($25.00). Existing SIUs shall apply for a permit within ninety days after the effective date of this chapter, and proposed new SIUs shall submit the standard industrial waste survey that is currently being used by the City.
   The City shall evaluate the data furnished by the SIUs and may require additional information. After evaluation and acceptance of the data furnished, the City may issue a wastewater contribution permit subject to terms and conditions provided herein.
   (c)    (1)    Within 180 days after the effective date of a categorical pretreatment standard, or 180 days after the final administrative decision upon a category determination submission under 40 CFR 403.6(a)(4), whichever is later, existing industrial users subject to such categorical pretreatment standards and currently discharging to or scheduled to discharge to the Bellevue sewage system shall be required to submit to the Safety-Service Director a baseline monitoring report (BMR) as outlined in 40 CFR 403.12(b)(1-7).
      (2)    Where reports containing this information have already been submitted in compliance with 40 CFR 128.140(b), duplication shall not be required.
      (3)    New sources, and existing sources that became industrial users subsequent to the promulgation of an applicable pretreatment standard, shall be required to submit a BMR at least ninety days prior to the commencement of the discharge. Additionally, new sources shall provide prior to discharge, information on pretreatment methods they intend to use and data on production, flow and concentrations of regulated pollutants. This information shall be supplied in accordance with 40 CFR 403.12b(1-5).
   (d)    Reports submitted by industrial users shall bear the signature of an authorized representative of the industrial user. Authorized representatives shall be:
      (1)    Principal executive officer (vice president level) if IU is a partnership or sole proprietorship, or
      (2)    A duly authorized representative of the individual designated above if the representative is responsible for the overall operation of the facility from which the indirect discharge originates (plant manager).
