Application for licenses for taxicabs or public conveyance vehicles for hire, shall be made to the Mayor, and shall set forth the name and address of the applicant, the trade name under which the applicant does business, the make, year, model, power and State license number of the vehicle for which the license is desired, as well as the color scheme and lettering and marks to be used on the vehicle, and credentials showing that the owner or operator has a definite and established place of business equipped with such facilities as will enable the owner or operator to keep such record of incoming calls as are required and such other information required by the Mayor.
