§ 155.069 APPLICATION FOR PERMIT.
   Application for a street graphic permit shall be made on forms provided by the Building Commissioner, and shall include the following information:
   (A)   Name, address and telephone number of the applicant;
   (B)   Location of the building, structure or lot upon which the street graphic is to be erected;
   (C)   Type of establishment which will display the graphic;
   (D)   Position of street graphic in relation to nearby building, street grade and traffic-control devices;
   (E)   Two copies of plans and specifications showing method of construction, location and support;
   (F)   Sketch showing street graphic faces, exposed surfaces and proposed message accurately represented in scale as to size, proportion and color;
   (G)   Name of person, corporation or firm erecting the street graphic;
   (H)   A copy of stress sheets and calculations showing that the structure is designed for dead load and wind pressure in any amount required by this and all other laws and city ordinances, if required by the Building Commissioner; and
   (I)   Such other information as the Building Commissioner shall require to determine full compliance with this and all other laws and city ordinances.
