(A) Every applicant for a license shall apply in writing to the Mayor.
(B) He or she shall set forth in such application his or her name and, if the application is made by or on behalf of a co-partnership, the names of all the members together with the style of such co-partnership, and the names of the sureties if a bond is required. If the applicant has a permanent or fixed place of business, it shall be stated in the application.
