Any person eligible for and receiving benefits under PSEBA shall at least annually complete and deliver to the Office of Human Resources a form provided by the city certifying the nature and extent of any continuing eligibility for such benefits, including but not limited to identity of spouse and dependents, current employer(s), and other health insurance available. Such certification shall be made subject to Section 10(a)(2) and 10(a)(3) of the PSEBA (820 ILCS 320/10(a)(2), (3)).
