(A) Production casing of a size not less than four and one-half inches outside diameter, in good condition, shall be set no higher than the top of the producing formation and cemented with a sufficient amount of cement to obtain a minimum of 500 feet of annular fill above the casing.
(B) All of such production casing shall be tested by pressure methods for either drilling the cement plug or perforating, if the cement is not drilled.
