Application for a contractors' license shall be made to the village clerk in writing, signed by the applicant if an individual, or signed by an authorized individual if an entity, verified by oath or affidavit and shall contain: a) the name of the applicant, b) the type of license desired, c) the time covered, d) the fee to be paid, e) the address to which any notice provided herein is to be mailed, and f) a listing of insurance coverage then in effect and which applicant agrees to keep in force throughout the period to be covered by the license, including workers' compensation, public liability for personal injury or death, property damage and vehicle coverage, listing the insuring company, policy number, amount of coverage and expiration date. At least the minimum limits of insurance shall be as required by law for workers' compensation; five hundred thousand dollars ($500,000.00) for general liability for both public liability and vehicle coverage. The application shall also contain any other information as may be required by the village. The procedure for approval contained in section 3-1-6 of this title shall apply to contractors' licenses.
