§ 91.17 APPLICATION REQUIREMENTS.
   An application for a sewage management permit shall include the following items:
   (A)   Two completed copies of the application;
   (B)   Two copies of the approved subdivision, land development or plot plan;
   (C)   Two copies of the BCDH sewage disposal system permit, DEP clean streams permit or other applicable permits from BCDH or DEP, including a copy of the application for the permit with all approved maps, plans and narratives attached, and a written narrative report including or prepared in accordance with the following:
      (1)   General project description;
      (2)   Location map;
      (3)   Site characteristics;
      (4)   Soils based on the most recent U.S.D.A. approved soils map;
      (5)   Projected sewage flows;
      (6)   Water supply;
      (7)   Sewage facilities alternatives analysis;
      (8)   Environmental considerations; and
      (9)   Project financing.
   (D)   Where required, an O&M agreement, in a township-approved form, that clearly sets forth the operation and maintenance responsibility for all components of the sewage system, including the following:
      (1)   Description of the method and extent of the maintenance requirements;
      (2)   When maintained by a private entity, identification of the individual, corporation, association or other entity responsible for operation and maintenance;
      (3)   When maintained by a private entity, provisions granting the township the right to:
         (a)   Inspect the facilities at any time;
         (b)   Require the private entity to take corrective measures and assign the private entity reasonable time periods for any necessary action; and
         (c)   Authorize maintenance to be done and lien the cost of the work against the properties of the private entity responsible for maintenance.
      (4)   Establishment of suitable easements, where applicable, for access to the sewage system.
   (E)   Sewage management permit fee in the amount established by the Township Board of Supervisors. The permit fee schedule is subject to change from time to time by resolution of the Board of Supervisors; and
   (F)   Construction escrow and/or maintenance escrow, when applicable.
