(A) Card Security. Each department needs to determine how to secure the cards. In some areas it is more appropriate for individuals to carry the cards. In others, the cards are stored in a secure place in the office. In either case, the credit card number should be protected as any blanket, open vendor order number, or your own personal credit card has been in the past. Cards must only be used by the cardholder appearing on the card.
(B) Canceling a Card. To cancel a card, the card should be cut in half, and forwarded to the Clerk-Treasurer's Office. The original Cardholder Agreement Form will be signed by the Clerk-Treasurer's Office to record proof of the card return. For immediate cancellation, contact the Clerk-Treasurer's Office by phone, fax or email. (See Appendix B, as attached to Ord. 13-2013, passed 6-11-13.)
(C) Lost or Stolen Card. If a Procurement Card is lost or stolen, immediately notify: the provider by calling the phone number on the back of the card. You can call this number 24 hours/day, 365 days/year. Notify City Police (275-3311). Also, notify the Clerk-Treasurer's Office (275-1606). Advise if you will need a replacement card. A new cardholder's agreement will need to be presented. Card replacement will take 3-5 working days. If you desire 24-hour priority processing, please be aware there could be a charge for this service.
