(A) Licenses shall be issued by the Bedford Police Department, upon the applicant’s complying with the following provisions.
(1) Giving his name, present address, and his permanent address.
(2) Giving a brief description of the nature of the business and the kinds of goods, property, services, or publication to be peddled or solicited.
(3) Paying the license fee required by § 111.12 for the license.
(4) Giving the name and address of the licensed employer and exhibiting credentials showing he is an authorized employee or representative of the licensed employer, in the event the application is for an employee’s license.
(5) Furnishing two full-face photographs of the applicant, two inches by two inches in size, one of which shall be attached at all times to the license, and the other delivered by the Clerk-Treasurer to the Chief of Police for his files.
(6) Stating that he has not been convicted of committing any felony or crime involving moral turpitude.
(B) All information required by divisions (A)(1),(2),(4), and (6) above shall be set forth in a verified written application which shall be subscribed and sworn to by the applicant before an officer duly authorized by law to administer oaths.
