1521.03 NOTIFICATION OF UNAUTHORIZED MATERIAL SPILL, RELEASE OR DISCHARGE.
   Upon the occurrence of an unauthorized material spill, release or discharge within the City of Bedford heights the party in possession of the unauthorized material at the time of the spill, release or discharge, whether he be the owner, an employee of the owner, an agent of the owner or such other party there having possession of the unauthorized material, shall immediately notify the Community Emergency Coordinator of the local Emergency Planning District, the Bedford Heights Fire Department, the Environmental Protection Agency (federal and state) and the Regional Sewer District pursuant to the Ohio Revised Code Section 3750.06. Such notice shall include the following:
   (a)   The location of the release (spill or discharge).
   (b)   The chemical name or identity of any substance involved in the release (spill or discharge) and whether the substance is an extremely hazardous substance.
   (c)   An estimate of the quantity of any substance released into the environment.
   (d)   The time and duration of the release (spill or discharge).
   (e)   The environmental medium or media into which the substance was released.
   (f)   Any known or anticipated acute or chronic health risks associated with the release (spill or discharge) and, if known to the informant, advice regarding medical attention necessary for individuals exposed to the substance released.
   (g)   Proper precautions to take as a result of the release (spill or discharge), including evacuation and other proposed response actions, unless that information is readily available to the Community Emergency Coordinator, pursuant to the plan of the district prepared under Ohio R.C. 3750.04.
   (h)   The name and telephone number of the person or persons to be contacted for further information.
   (i)   Such other information as may be required by rules adopted under Ohio R.C. 3750.02(B)(1)(F).
