1177.03 PROCEDURE FOR OBTAINING PERMIT.
   (a)    Original application. Any person seeking permission to conduct a child care center shall file an application with the Building Commissioner who shall examine the property to determine if it is suitable for use according to the building, sanitation and fire laws of the city and the state and, if meeting such laws, shall forward the application along with the Building Commissioner's report to the Planning Commission for review. The fee for filing an application or renewal for a child care center shall be two hundred dollars ($200.00).
   (b)   An applicant for a permit to operate a child care center shall submit the following written information to the Planning Commission:
      (1)   Information explaining the need for the child care center, the clientele to be served and the financial resources available to operate the facility.
      (2)   A license authorizing operation of a child care center from the Director of Public Welfare, State of Ohio. (in no instance will the Planning Commission consider renewing a permit for a child care center if a license has not been granted to operate such facility within a 365-day period commencing at the time a permit was originally approved.)
      (3)   A detailed plan of services and/or programs to be offered the children utilizing the child care center.
      (4)   The name(s) of the individual(s) and/or agency responsible for administering child care services.
      (5)   Evidence that use of the proposed site will not adversely disrupt the surrounding neighborhood from the standpoint of noise, lights, congestion and/or traffic generation.
      (6)   The proposed hours of operation of the child care center.
      (7)   The number of children anticipated to be in average daily attendance at the child care center.
      (8)   Evidence that both fire prevention and property maintenance inspections have been made of the structure proposed to contain the child care center, as well as evidence that such building is free of any violations of local and/or state laws or codes.
      (9)   Plans and drawings which show the location and dimensions of the lot, the location and dimensions of any existing or proposed structures on the lot, floor plans, points of ingress and egress, landscaping and/or screening areas, recreation and open space facilities, and a parking and traffic circulation plan.
      (10)   Evidence of insurance adequate to cover the cost of fire, theft, vandalism and liability.
      (11)    Evidence that the child care administrator has been examined, within the past ninety days, and annually thereafter, by a licensed physician, and has been certified to be free from communicable disease.
      (12)    Evidence that the child care administrator has either been trained in first aid by a doctor or nurse, has completed a course in first aid which is approved by the State Department of Health, or will assume responsibility for ensuring the presence, at all times, of a person who has such knowledge or training.
