157.14 HEALTH INSURANCE.
   (a)   The City will make available group health insurance benefits for employees for single or family coverage pursuant to an annual contract entered into by the City of Bedford Heights with a duly-qualified health insurance provider. The benefits, deductibles and other terms and conditions of said health care coverage shall be delineated in the annual contract between the City and the health care insurance provider.
      (1)   All eligible employees shall contribute eight percent (8%) of the premium for family health insurance and eight percent (8%) of the premium for single health insurance. However, employee contributions shall not exceed ninety dollars ($90.00) per month.
      (2)   The City shall create a Section 125 plan, which will permit the City to make the contributions on a pre-tax basis.
   (b)   All full-time employees and the Director of Law, Prosecuting Attorney, and Assistant Director of Law/Assistant Prosecuting Attorney shall be eligible for group health benefits. Each person who desires to participate in the group health benefits program must make arrangements with the Director of Finance for the applicable plan.
   (c)   The City will provide to all eligible employees the Health Care Benefit Plans, Dental Insurance and Vision care benefits with employee contributions and deductibles as provided for in the health care coverage plan delineated in the annual contract between the City and the health care insurance provider.
   (d)   Newly-hired employees will be provided group insurance benefits upon completion of their insurance enrollment period or a period of three months of continuous active service, whichever comes first.
