§ 112.04 PERMIT APPLICATION.
   (A)   Any person who shall desire a temporary food sales permit shall make application therefor in writing to the Borough Office. Such application shall be accompanied by such application fee, as required by a schedule of fees, established by and amended from time to time by resolution of Borough Council. Such application shall set forth and include the following information:
      (1)   The name, business name, and address of the applicant and the property owner, if different than the applicant;
      (2)   Contact information, including phone number and e-mail address of the applicant and the property owner, if different than the applicant;
      (3)   If a vehicle is to be used, vehicle information, including business license number, truck make and model, and license plate number;
      (4)   Temporary food sales information, including types of goods to be sold and proposed hours of operation;
      (5)   Copy of the state health certificate under the Retail Food Facility Safety Act, 3 Pa.C.S.A. § 5701-5714, or other applicable statutes;
      (6)   Written consent of the property owner, if different than the applicant; and
      (7)   Such other information as may be required from time to time by the borough.
   (B)   No action shall be taken on any application for a permit under this section until the application has been completed in its entirety and the application fee, as required by a schedule of fees, established and amended from time to time by resolution of the Borough Council, has been paid in full. There shall be no proration of fees under this division (B).
