Any licensed veterinarian who inoculates a village resident's dog(s), cat(s) or animal against rabies in Lake County shall enter such evidence in a legible manner on a standardized form approved by the health department. Veterinarians who inoculate a dog or cat for rabies shall procure from the Lake County health department numbered tags, one to be used with each inoculation certificate. Only one dog or cat shall be included on each certificate. Veterinarians may charge pet owners a service fee not to exceed ten dollars ($10.00) for each registration tag issued. All registration certificate tag fees shall be submitted to the animal rabies control program monthly. (Ord. 2007-O-84 § 2)
