5.16.205: INCIDENT REPORTING:
   A.   Each licensee shall maintain a system for recording unusual incidents. Incidents that must be recorded include, but are not limited to:
      1.   Escorted removal of a patron with physical contact by the licensee or licensee's agent(s) or employee(s);
      2.   Injury of a patron (must be reported to the Lake County sheriff's department immediately);
      3.   Overintoxication of a patron that has resulted in licensee/agent/employee assistance or medical or police intervention;
      4.   Physical altercations involving punching or striking (must be reported to the Lake County sheriff's department immediately);
      5.   Employee/agent/licensee use of force (physical contact with a patron that exceeds the contact necessary to guide or direct the patron);
      6.   Call for police or fire assistance;
      7.   Display or use of a weapon including, but not limited to, guns, knives, pool cues, beer bottles and projectiles (must be reported to the Lake County sheriff's department immediately);
      8.   Criminal damages to property or theft (must be reported to the Lake County sheriff's department immediately);
      9.   Patron complaint of licensee/agent/employee conduct.
   B.   The term "immediate" or "immediately" as used in this section means as soon as licensee/agent/employee has knowledge of the incident.
   C.   Each incident shall be recorded on the same business day on which it occurred, and shall include the names and addresses of the patron(s) involved, if known, the licensee/employees/agents involved, the manager on duty and a brief description of the incident.
   D.   The incident reports shall be produced to the local liquor commissioner within seven (7) days from the date of the occurrence of the incident. (Ord. 2010-O-15 § 2)
