5.04.040: LICENSE; COOPERATION AGREEMENT:
The applicant by submitting the application for the purpose of receiving an amusement hall for minors license agrees to cooperate with the mayor or his designated representative by providing information concerning the applicant's character and reputation and prior criminal background. (Ord. 1990-O-42 § 1)
