5.02.020: CERTIFICATE APPLICATION; CONTENTS:
An application for a business registration certificate shall be made on a form to be furnished by the village and shall be filed in the office of the village clerk. The application shall, at a minimum, include the following information: name of business, business location address, business mailing address, business telephone number, owner of business, owner's mailing address, primary contact person and title, residential and business telephone number of the primary contact person, type of business and other such information as may be determined necessary by the village. If the applicant is a corporation, the application shall contain the name and residential and business telephone numbers of each principal officer and the registered agent. (Ord. 2007-O-34 § 2)
