(A) Applications for water, sewer, and other municipal utility services shall be made to the Clerk/Treasurer. When a water service connection is to be made, the application may be made by the owner or the agent of the owner, or by the lessee, the occupant, or the person in charge of the premises.
(B) The owner, lessee, occupant, or other person in charge of any premises, and the applicant for any municipal utility service, shall provide the city with such information as may reasonably be required relating to the provision of or the use of municipal utility services upon the premises. No person shall willfully fail to provide such information or willfully falsify such information.
