(A) All pertinent forms shall be provided by the Department. No permit shall be issued for construction unless the application for the permit is accompanied by:
(1) The payment of appropriate fees as described herein;
(2) Plans and specifications showing the construction to be accomplished; and
(3) In the event of construction of a new structure, or extension to an existing structure, a plot plan showing the location of the new structure or the extension to existing structure, as well as all other existing improvements and lot boundaries.
(B) (1) Upon application to the Department, the applicant may request a waiver of the aforesaid plot plan for good cause shown or undue hardship shown.
(2) A waiver of same may be granted and in the sole and absolute discretion of the Chief Code Enforcement Officer; provided, the waiver is not contrary to the rules, regulations, codes, revisions, amendments or supplements as set out in § 150.06.
(C) In addition, a copy of a design release, issued by the State Building Commissioner and the State Fire Marshal pursuant to I.C. 22-15-3-1, shall be provided to the Chief Code Enforcement Officer before issuance of a permit for construction covered by the design release.
