Upon application for a license required by § 832.01 of this chapter, the applicant shall make a full disclosure under oath of the following information:
(A) The true name of the owner of the goods, wares or merchandise to be offered for sale and the name of the operator of the sale if he or she is a person other than the true owner; and
(B) A full and complete inventory of the quantity, kind and character of the articles to be offered for sale, which inventory shall be prepared in the following manner:
(1) An itemized listing of all articles and the quantities thereof remaining unsold which have been in stock on the premises where such sale is to be conducted for a period of 15 days or longer prior to the application for the license;
(2) An itemized listing of all articles and the quantities thereof received in stock on such premises within the 15 days immediately preceding the application for the license; and
(3) An itemized listing of all articles and the quantities thereof which have been ordered to be placed in stock at such premises for the duration of the sale, and the reason for the urgent and expeditious disposal thereof; provided, however, that, all articles itemized as provided in this section shall be goods, wares or merchandise purchased by the applicant for resale on bona fide orders without cancellation privileges and shall not comprise any articles purchased on consignment.
