(A) Any massage establishment or massage practitioner desiring to provide out-call massage services shall submit to the Chief of Police an application to provide out-call massage services within the city. The application shall be accompanied by a nonrefundable filing fee established by separate resolution of the city council to help defray the cost of the investigation required by this chapter. In addition to the requirement herein pertaining to an operator's permit and/or massage practitioner permit application, the applicant shall also submit detailed information, on the form provided by the police department, setting forth the manner and means of transporting, to and from the premises where out-call massage services are to be performed; clean, sanitary towels, coverings, linens and sterilized instruments to be utilized, as well as any supplementary aids, equipment or devices to be utilized and the method(s) of disposal thereof.
(B) The application and fee required pursuant to this section shall be in addition to any other license, permit or fee required by any other chapter of this code or ordinance hereafter adopted.
