§ 92.19 APPLICATION.

   An applicant for a permit under this subchapter shall, before using fireworks, file with the city a completed permit application showing the:

   (A)   Pyrotechnist’s:

      (1)   Name, contact persons and business address;

      (2)   Proof of legal competency; and

      (3)   Record of previous experience with fireworks;

   (B)   Location of the proposed display;

   (C)   Amount, type and class of fireworks to be used;

   (D)   Name and address of the company supplying the fireworks;

   (E)   Date of proposed display;

   (F)   Starting and ending times of the proposed display;

   (G)   Diagram of the proposed display grounds, detailing:

      (1)   Firing points;

      (2)   Location of buildings and highways on or adjoining the grounds;

      (3)   Spectator restraining lines; and

      (4)   Overhead obstructions;

   (H)   Provide the surety bond or insurance coverage required by state law for operators of fireworks displays.
