1066.02 FEES; AREA OF OPERATIONS; EFFECTIVE DATE.
   The fees set forth herein shall be established and implemented for the following Emergency Medical Services originating from anywhere within the City's corporate limits effective with the date of passage of this section:
   (a)   Advanced Life Support (AALS@): $750.00 per patient transported plus $14.00 per loaded mile from the location of patient pick-up to the hospital.
   (b)   Advanced Life Support II (AASL@): $875.00 per patient transported plus $14.00 per loaded mile from the location of patient pick-up to the hospital.
   (c)   Basic Life Support (ABLS@): $550.00 per patient transported plus $14.00 per loaded mile from the location of patient pick-up to the hospital.
   (d)   EMS Response Without Transportation: there will be no charge.
   (e)   The City shall bill the patient or the responsible party for any emergency medical services rendered in accordance with State or Federal guidelines. The Mayor is authorized to execute any and all documents and/or to implement any procedures necessary to effectuate the collection of such fees. For Emergency Medical Services rendered by the City pursuant to an existing mutual aid contract with another jurisdiction, the EMS fee, if any, for services rendered by City of Avon personnel shall follow the EMS fee policy, if any, in effect for the non-City of Avon jurisdiction. The Director/Chief of the City Fire Department may, in conjunction with the Director of Finance, make a recommendation to the Mayor no more than once per year to adjust the EMS fees in accordance with the Usual, Customary and Reasonable (UCR) fee schedule as may be established from time to time by either a State or Federal agency, or such recognized insurance trade association, as may be recognized or otherwise authorized by law to establish such UCR fee schedule.
