(A) Application for an original campground permit shall be in writing and verified and shall contain the following:
(1) The name and address of the applicant;
(2) The interest of the applicant in the campground and its location;
(3) The legal description of the real estate; and
(4) A complete plan and supplemental specifications (in triplicate) of the campground showing compliance with all applicable provisions of city ordinances, including this code of ordinances.
(B) An application for a renewal of a campground permit shall be required and the renewal issued by the City Clerk-Treasurer before any changes are made in the campground which deviate from the plans and specifications as approved in the original permit or the latest renewal thereof. Applications for renewals of permits shall be in writing and verified by the holder of the permit and shall set out any proposed changes in the campground.
