§ 51.37  PROTECTION DEVICES.
   (A)   The costs of purchasing, installing and maintaining a backflow device shall be the responsibility and sole expense of the customer. The installation of each backflow prevention device, except for outlet fixture vacuum breakers, shall be by a licensed plumber. The customer shall make application to install a required backflow prevention device to the Water Commissioner on a form provided by the city. The application shall contain the name and address of the applicant, the type of potential backflow protection and type of device to be installed, including brand and model number.
   (B)   Annual testing of all double check values and reduced pressure zone or pressure type vacuum breakers shall be performed by a certified Grade VI tester. If maintenance or repairs are necessary, the user shall be issued an order by the Water Commissioner to make all repairs or maintenance.
