It shall be the duty of the customer at any premises where backflow prevention devices are installed to certify to the City Water Department at least one time annually, or at more frequent intervals if required by the Manager, that the backflow prevention device has been tested by a State Department of Health and Human Services Grade VI license holder if the device is equipped with test ports. Certification shall be made on a form available from the City Water Department. Testing shall be at the expense of the customer. In the event the customer fails to certify the testing to the City Water Department as required, the City Water Department will, after notice, test any backflow prevention devices and bill the customer. The rate for testing by the City Water Department shall be designated by resolution of the Board of Public Works. If the customer fails to pay the City Water Department for testing, water service shall be discontinued after due notice to the customer.
